Address, facility type, utility availability, electrical, water, and drainage access.

How ice is currently supplied, monthly volume, and existing challenges.

Traffic & Operational Needs

Daily foot traffic, seasonal demand, and installation size.

Site photos and any additional context about your location.

Facility Address

Street address of both your installation and the proposed kiosk location.

Electrical Information

Whether a dedicated 208–230V (220V) circuit is available near the proposed location.

Water & Drainage Access

Whether a cold water supply line and floor drain are accessible within 15 feet of the proposed spot.

Site Photos

Phone photos of the proposed location, nearest outlet, water source, and surrounding area.

Contact Information

Name, title, phone, and email for the primary and secondary point of contact.

Ice Program Data

Current ice supply method and approximate monthly bag volume for your location.

What Happens Next

Your survey will be reviewed by the PIP operations team. Here's what to expect after submission.

Survey Review

The PIP team reviews your submission for completeness and confirms that the proposed location meets minimum site requirements.

Site Assessment

A PIP site inspector may follow up to verify utility access and confirm installation feasibility at the proposed location.

Deployment Approval

Approved locations are added to the deployment schedule. The PIP operations team will contact your primary point of contact directly.

Installation

PIP coordinates delivery, installation, and utility connection. Your team's involvement during installation is minimal.
